How to Become a BMS Approved Carrier
Thank you for your interest in becoming an approved carrier for BMS, Inc. To complete the process we need the following documentation:

Carrier Profile – You start by opening a BMS Carrier Account on this page. Then complete the profile form. Once you’ve opened an account you don’t have to complete the whole profile because you can log back on and complete it later. Just remember we can’t dispatch and pay you until we have everything.

FMCSA Operating Authority – Fax or upload a copy of Federal Motor Carrier Safety Administration Operation Authority.

W9 Tax Payer ID Form – Fax or upload a completed IRS W9 Form.

Certificate of Insurance – Fax or upload a certificate of insurance naming BMS, Inc as certificate holder – with the original certificate to follow. The certificate must have the following minimums:

$ 1,000,000.00 in Auto and $1,000,000.00 General Liability

$ 100,000.00 Cargo Minimum with Reefer breakdown coverage
We can approve you for dispatch with a faxed or uploaded copy of your insurance certificate but cannot approve you for payment until we receive the original cert.

Completed and Signed Contract – Log on to the BMS, Inc. website then download a contract from the My BMS Carrier page. When signed you may upload it or fax a copy but we will need a signed original BEFORE we can dispatch and pay.
